All new dogs must pass a FREE interview and trial day before participating in daycare at Modern Dog. Please complete and submit the Pet Registration Form prior to scheduling your interview. Interviews are scheduled by appointment only through the form below.

ALL DOGS MUST MEET REQUIREMENTS (for daycare only)

• At least four months old

• Spayed or neutered (if six months or older)

• Current on Rabies, Distemper, Parvo, and Bordetella vaccines

• Flea/tick free and in good overall health

• No open wounds or stitches of any kind

• Wearing a quick-release collar and name tag

• Dog and people friendly

• Able to pass the interview and trial day

Please complete the interview request form below. PLEASE NOTE: This is a request only. You will be contacted shortly by Jordan to schedule the actual time for the interview. This takes place in your home and last approximately 30 minutes. If we sniff each other and decide your dog is a good candidate for daycare, we will schedule the first visit.
